SIGNING UP FOR A FREE FOOD BOX ON-LINE
• Fill out the form on this page. It must be complete.
• Send the form by e-mail to: us_moms@yahoo.com Mark the e-mail as urgent if you are
in desperate need for food right away OR print it out and fax it to 604-261-2282
• Next, call the office at 604 261-2282 to get a start up date. Leave a message if no one
answers when you call. A staff person will get back to you within a few days. If you are
urgently in need of food, again make sure you clearly state that in your message.
• REMEMBER:
It could take several days to contact you. We have not forgotten you. We do not always
have enough volunteer staff to process these calls as quickly as we would like to be able
to. You can call again if you have not heard from us by the third day.
• REMEMBER:
We need to know how to contact you, so be sure you include your name and a phone
number. We do not contact people by mail. You can use your e-mail address for your
contact method if you do not have a phone.
• REMEMBER:
If your contact number EVER changes, be sure to send the new information by e-mail to:
us_moms@yahoo.com
Be sure you indicate it is a change in contact number by referring to your previous contact
number. For example the e-mail should read like this:
My name is …… I am contacting to notify you of a change in my contact
number. I applied to you on November 1, 2002. My old number was (604)
222-1313. My new contact number is (604) 321-1222. I can be contacted
there by leaving a message.
